YMHDD is committed to developing innovative programs and expanding services, such as providing transportation for our aides to enable us to serve hard-to-reach areas and a shopping service for our for our consumers. Our goal is to enable the physically challenged, the elderly in need, and others confined to their homes to maintain their independence.
In-Home Services: In-home services are provided on an outreach basis to work with individuals and their families in the home and community. They are designed to avert treatment in residential or inpatient settings or to facilitate the earlier return of individuals reciving inpatient or residential care. Services are multi-faceted and include therapy, supportive counseling, skills training and facilitation of access to other needed services and supports. Some in-home approaches are limited to short-term, intensive approaches with the goal of stabilizing and connecting the individual and family with on going services intensity varying with clinical needs. Parent Advocate Services: for parents who are developmentally disabled, parenting skills, and pregnant woman issues to provide a safe and nurturing environment for their children to grow

Community Living Homes:
CONCEPT: Consumers living in a living in a Licenced Community Residence Developmental Disabilities under contract will be refferred for placement by DDD. The community residence will be open 24 hours a day, 365 days a year. Residents are routinely expected to be present seven days a week in order to fully benefit from the training provided by the program.

We are qualify for provider managed supports and Self- Directed Services for Real Life Choices, participants of Real Life Choices, purchase services and supports based on an individual and family bugdet, to achieve the outcomes they have identified in their Plan of Care.
